In order for the Central Valley Community Foundation to process your scholarship award, the following information and documentation must be submitted. Upon receipt and verification of your materials, your scholarship check will be made payable to you and the institution. The check will be mailed to you. You will need to take the check to the financial aid office at your school to apply it to your account. If for any reason you choose not to collect all or a part of your scholarship award, please contact us at email@example.com.
Scholarship Verification and Attachments
1. Verification of Enrollment (Class Schedule with Credits Listed)
2. If this is verification for a second payment, include a transcript from last semester with grades
An attachment of verification of full-time enrollment this can be a PDF of your class schedule or unofficial transcripts
If you are receiving a multi-semester/year scholarship, also include a grade report showing a GPA for full-time attendance during the previous semester
